Bagi graduan yang tidak berkesempatan mengambil diploma dan transkrip rasmi tersebut atau tidak menghadiri majlis konvokesyen boleh mengemukakan permohanan tuntutan diploma dan transkrip dengan mengisi borang berikut dan datang sendiri ke KYPJ :
